We are currently seeking per diem RN or LPN to provide in-home medical services to our clients.
RN or LPN works under the direction and supervision of the RN case managers.
Job responsibilities include but are not limited to the following:
direct patient care in a home environment; implementation of physicians prescribed plan of care; performance of treatments and therapies; patient assessments, coordination of care; health promotion, and continued teaching, and training of patient and/or family members.
Our ideal candidate will have at least 1 year of med-surg experience preferably in home health care, have excellent communication and documentation skills and have CPR certification.
